We are committed to ensuring that the cost of services is never a barrier to the care your family deserves.

What is the Sliding Fee Program?

The Sliding Fee Program is a discount schedule that reduces the cost of your healthcare services based on your household size and annual income. It is designed to assist individuals and families who are uninsured, underinsured, or have high insurance deductibles. Our goal is to remove financial barriers to care, ensuring that your ability to pay never determines the quality of treatment you receive.​

How Do I Determine if I Qualify?

The Sliding Fee Discount Program is designed to ensure that financial barriers do not prevent you from receiving the care you need. Qualification is based on your total household size and gross income (total income before taxes). 1. Who is Eligible? You may qualify for the program if you find yourself in any of the following situations: Insurance Status: You are uninsured, underinsured, or have insurance that does not cover specific services. Financial Standing: You have limited income resources or are experiencing temporary financial difficulties. Medical Hardship: You are facing financial strain due to high medical expenses. Note: We welcome all patients regardless of employment, insurance, or ability to pay. Our services are provided without regard to race, ethnicity, age, gender, sexual orientation, religious beliefs, or political affiliation. 2. Required Documentation To finalize your eligibility, you must provide proof of gross income for all household members. Acceptable forms of proof include: Tax & Employment Records: Previous year’s federal tax return, 2–4 recent paycheck stubs, or a signed letter from an employer on company letterhead confirming gross wages. Benefit Verifications: Unemployment compensation, disability benefits, worker's compensation, Veteran’s or Survivor’s benefits, and pension or retirement income. Support & Assistance: Alimony, child support, educational assistance, or a notarized letter from a third party confirming how they are supporting you. Other Income Streams: Interest, dividends, rental income, royalties, estate funds, supplemental income, and any other sources of household funds. 3. Ready to Apply? If you meet any of the criteria above, we encourage you to apply. Our staff is available to help you gather your documentation and walk you through the application to ensure your care is affordable.

How Do I Apply?

Estimate Your Discount before applying, you can use the Poverty Guidelines Table, Page 2 (Dollars per month) to estimate your potential savings:

  1. Find your household size in the left-hand column.

  2. Locate your annual income range on that same line.

  3. Look at the top of that column to see if you qualify for a nominal fee or a specific percentage-based discount.

  4. Determine your cost: Once you have your percentage, refer to our Counseling Sliding Fee chart to find your exact Session Fee based on that income bracket.

Note: This tool provides an estimate only. Final eligibility is determined upon review of your completed application.

Step 2: Submit Your Application

  • Request an Application:
    Email us at admin@norwaymn.com with any questions or to have an application sent to you.

  • Apply Online:
    Click here to access the application form directly.

  • Finalize: Send your completed application along with the required proof of income to admin@norwaymn.com. Our team will review your documents and contact you with a final approval.

How Often Do I Need to Apply?

All participants must re-apply annually and provide current proof of income each year.

About the Sliding Fee Chart

  • The schedule is updated annually according to the Department of Health and Human Services.

  • For families with more than 6 members, add $5,140 per additional person to the income calculation.
